This document provides details of Core Rehab’s Privacy Policy. This policy relates to our use of personal information collected from you by either in person, by letter, by email, by telephone, social media or through our website. At Core Rehab we value the importance of protecting your privacy and keeping your information secure in line with the General Data Protection Regulations (GDPR).

This policy may be updated from time to time and the latest version will be published on our website.

Personal Information

At Core Rehab we will collect personal information so that we can support your treatment process or gather relevant information to handle your enquiries. We also have a legal obligation to do so and must ensure that the data is relevant, adequate, and limited to what is necessary.

The personal information that we collect from you may be (but not limited to);

• Full name
• Date of birth
• Address
• Email address
• Contact telephone number
• Occupation
• Source of referral
• Account details that relate to payments for our services
• Background to your condition
• Medical history
• Why you feel you require Physiotherapy and/or Pilates.

We will also document your assessments and treatments on paper or electronic notes as you continue to be seen by us.

We may collect this information in various ways depending on the situation:

• Over the telephone – collecting personal information, responding to a general enquiry from a client and carrying out a triage to see if they are suitable for our service.
• Through our website – responding to a client when they complete an enquiry through our “contact” page
• Via email – responding to a client’s enquiry.
• • Via Facebook – responding to a client’s enquiry.
• Paper notes – to document assessments and treatments which are kept in secured, locked filing cabinets.

How we use your personal information

We use the information that you give us to understand how your condition is affecting you, what your needs are, and to ensure that we can tailor our treatments to best suit your needs and goals. It also allows us to provide you with a safe and effective service.

We will also collect personal information to obtain payment for our services. Copies of the invoices are also kept electronically on Cliniko for accounts and tax purposes.

With your permission we may pass your information on to other health professionals who are involved in your care e.g. your GP or consultant. This information may be given in a form of a letter. If this is the case, the letter and the protection of its contents becomes your responsibility.

We may also use your details as part of our audits to help us to improve our treatments and services to individuals like yourselves.

We may use your personal information to contact you in the future about promotions we think you may be interested in e.g. a new Pilates class.

Payments

We will also collect personal information to obtain payment for our services. Copies of your invoices will be stored electronically on Cliniko for accounts and tax purposes.

Updating and controlling your personal information

You have certain rights under the General Data Protection Regulation (GDPR) to access the information we hold about you. Unless we are required by law to reject your request, we can provide you with a copy of your information. A request for this information must be made in writing and processed by Core Rehab in 40 days.

We have a legal obligation to retain our client records for a period of eight years after the last point of contact. This is in accordance with CSP and HCPC professional guidelines.

We will not share or distribute your personal information to third parties unless we have your prior consent or are required by law or court order to do so.

It is your responsibility to contact us to update your details if they change. We welcome any queries, comments or requests you may have regarding this privacy policy.
